Educational Verifications for Licensure and Employment
State Medical Boards (such as the Georgia Composite Medical Board) and hospital credentialing offices often require a specific Verification of Education form to be completed by your graduating program.
- Process: Email your verification forms to tabitha.curtis@emory.edu at the PA Program office. Form K for Georgia licensure will be submitted following graduation upon completion of degree conferral by the registrar. All submissions are completed online. If you require copies, please send us a request.
- Official Seal: Our office will verify your degree conferral date, apply the official program/school embossed seal, and return the document directly to the requesting board or agency.
- Dean Signature/Notary required: To request completion of an enrollment verification form requiring SOM Dean signature or notary seal, please send an email, along with the form to the School of Medicine Registration team at somregi@emory.edu. Please allow 2-3 business days for processing.

Requesting Official Transcripts
The Emory Physician Assistant Program does not issue official transcripts directly. All transcript orders are handled through the Office of the University Registrar.
- How to Order: Alumni can order official electronic or paper transcripts online via Parchment.
- Delivery: Electronic transcripts (PDF) are the fastest method and are typically delivered within minutes.
- Forms with Transcripts: If you have a document that must be attached to your official transcript, you can upload it during the Parchment ordering process.

How much does a transcript cost? Emory University does not charge a per-transcript fee for standard delivery; however, third-party processing fees (via Parchment) or expedited shipping for paper copies may apply.
I graduated before 1990. How do I get my records? Alumni who attended prior to 1990 can still use the Parchment portal to request both paper and electronic/PDF transcripts, though processing may take slightly longer as records are retrieved from archives.
Does a transcript replace a "School Verification" form? Generally, no. Most State Medical Boards require a specific "Form K" or "Verification of Education" form completed and signed by the Program Director or Registrar in addition to your official transcript.
