EASIL Tools Line List Template

Data organization is key to effective prescription start measurement. EASIL has developed a line list template to help your facility begin the data collection and organization process, ensuring the most important measures are included and recorded appropriately.

Patient Outcomes

These are some outcomes that you can track to help improve important patient safety metrics.

Tracking C. difficile and antibiotic resistance

The National Healthcare Safety Network (NHSN) in 2012 launched the reporting component for nursing homes and LTCFs. Laboratory-based surveillance through NHSN of MRSA, E. coli, and others can serve as a proxy for infections.

Adverse Drug Events

Antibiotics can often have substantial, sometimes life-threatening side effects. Given that medications are responsible for a high proportion of preventable harm in nursing homes, and that antibiotics are the most commonly used medications in these patients, measuring and responding to adverse events is crucial to improving patient safety.

Financial Impact

Stewardship practices can both improve patient safety and reduce costs for unnecessary medication and costs related to infection control and outbreak response. Measuring the return-on-investment (ROI) of preventative measures can help to justify stewardship resource allocation in the future.
