Intended Learning Outcomes

  1. Identify potential patient care transitions (e.g., ER to inpatient team, ICU to floor, OR to floor, Outside hospital to Emory)
  2. Provide necessary information in a standardized format when handing off / transitioning care
  3. Formulate and effectively communicate a question for a consultant; understand the importance of bilateral dialogue on recommendations
  4. Describe the importance of care transitions and their impact on patient care
