- Career Development Conference Report: This form is used to prepare for and report on the faculty development conferences between faculty and their chairs.
- Chair Evaluation Form for Faculty Development Performance: This form is used annually when faculty are asked to review their chair’s performance on faculty development initiatives.
- Faculty Time Records: This form is used for monthly reporting of faculty activities, and serves as the documentation that enables Emory’s affiliated hospitals to reimburse the University for teaching and administrative services.
- Conflict of Interest: eCOI should be used by faculty to report external activities and the investigator’s financial interest in research, and to complete the annual certification process.
