To schedule your first appointment with the Department of Neurosurgery, please call 404-778-5770 and have the following information available:

• Patient's Name, Address and Telephone Number
• Date of Birth
• Social Security Number
• Diagnosis (if available)
• Name, Address, and Telephone Number of Treating Physician
• Insurance Information

We ask that you send medical records prior to scheduling an appointment to ensure you'll be scheduled to see the appropriate physician. Because we are super-specialized, our surgeons are experts not only in the specific areas of neurosurgery, but also specific procedures.  Record review allows our providers to ensure patients are getting to the best provider to manage their illness.  Before your first appointment, please:

1. Send medical records pertinent to your diagnosis to (including MRI):

       Please note, that due to patient confidentiality, we are unable to accept forms electronically
2. Please send or bring your MRI(s) on disk or film.  Without the MRI on the day of your appointment, the surgeon will not be able to properly evaluate your condition.
3. Prepare a list of questions and concerns to discuss with your medical team.
4. If you are a new patient, fill out a New Patient Packet in preparation for your first appointment.
