Registration Materials

To register for the Summer Science Academy, parents must submit all registration materials.  Space in the academy will not be reserved until all documents and fees are received.

The registration materials include:

1) Registration Form
2) Deposit ($100)
3) Photo consent release form
4) Copy of health insurance card or statement of financial responsibility
