Basic Brush Instruction
We have enclosed two sterile brushes for each person to be tested. Please make sure that each individual's brush set is kept separate to avoid sample mix-up. You may do the brushing any time, as long as you have not had anything to eat or drink recently. The best time is first thing in the morning before brushing your teeth. Please return the sample collection brushes and the consent form as soon as possible after completing the procedure.

Sample Collection Procedure:

Use two brushes per person -- one brush for each cheek.

1. Insert one of the brushes in the bottom of the space between the cheek and bottom gum.
2. Twirl the brush against bottom gum and cheek for approximately 30 seconds.
3. Place the brush back in the container tube and replace the top.
4. Repeat steps 1-3 with the second brush on the other cheek.

Once the Procedure has been Performed:

• Place all of the tubes back in the plastic bag and seal.
• Please read and complete the consent form.
• Place the consent form and the plastic bag containing the collection brushes in the self-addressed posted envelope and mail as soon as possible. If you have a questionnaire, please complete and include in the posted envelope.
